Healthcare in Uganda: Overview

Uganda’s healthcare system is structured as a pluralistic network of government, private, and not-for-profit facilities delivering services across the country. The Ministry of Health regulates both public and private providers to ensure standards and quality of care. Uganda lacks a comprehensive national health insurance system, and private insurance coverage remains low (approx. 5%). Most healthcare expenditure is funded publicly, with donor support and out-of-pocket spending still significant.

Despite progress in expanding access to primary care through Health Centre II, III, and IV levels, health system challenges persist — including uneven distribution of facilities, limited infrastructure outside urban centres, and frequent shortages in essential medical supplies.

Main Diseases and Health Challenges

Uganda faces a dual burden of disease: communicable diseases still cause high morbidity and mortality, while non-communicable diseases (NCDs) are rising steadily.

Communicable Diseases

  • Malaria — one of the leading causes of outpatient attendance and hospital admissions.
  • HIV/AIDS and Tuberculosis (TB) — historically high prevalence, requiring continued public health focus.
  • Ebola Virus Disease — Uganda periodically experiences outbreaks, including recent confirmed cases in Kampala that stressed the healthcare system.

Non-Communicable Diseases

  • Hypertension and Diabetes — increasing due to lifestyle changes and urbanization.
  • Neonatal and pediatric conditions — including pneumonia and anaemia — remain major contributors to inpatient mortality.

Mental Health

Mental health care receives a small portion of the health budget (~1%), leading to limited services beyond major referral hospitals.

Hospitals and Key Healthcare Facilities in Uganda

Uganda’s hospital network includes national referral hospitals, regional and general hospitals, private and specialized facilities, as well as community health centres.

Prominent Hospitals

  • Mulago National Referral Hospital — major tertiary referral centre in Kampala.
  • Kawempe General Hospital — public regional referral facility.
  • Kiruddu General Hospital — public general hospital in Kampala.
  • Uganda Martyrs’ Hospital Lubaga — large private not-for-profit hospital with community services.
  • Aga Khan University Hospital, Kampala — new tertiary facility expanding specialty care.
  • Nsambya Hospital Cancer Center — specialized cancer treatment centre under development.

In total, Uganda has multiple national and regional referral hospitals and a large network of district and community hospitals and health centres across urban and rural regions.

Uganda’s Healthcare Annual Budget and Trends

In the 2025/26 financial year, the health sector budget was set at UGX 4.353 trillion, an increase from previous years but still facing significant financing gaps due to reduced donor support and rising demand for services.

Although government allocates roughly 7–8% of national budget to health, stock-outs of medicines and supplies persist, and there is ongoing advocacy to strengthen domestic financing and implement universal health coverage through mechanisms like the proposed National Health Insurance Scheme.

Children’s and Women’s Health in Uganda

Children’s Health

Children in Uganda face risks from malaria, pneumonia, diarrhoeal diseases, malnutrition, and neonatal conditions. Ongoing immunization campaigns and outreach programs aim to reduce child mortality, but access issues and supply shortages still challenge outcomes, particularly in rural areas.

Women’s Health

Women’s health priorities include maternal care, family planning, reproductive health, and management of pregnancy-related complications. Uptake of family planning services has improved in selected regions, thanks to public health initiatives and partner support. Services continue to expand at Health Centre IV and hospital levels, although gaps in workforce and medicines remain.
